Determining Eligibility
We serve families from birth to end of life. The Intake and Eligibility process varies by age. Please check out our Eligibility Page for more information.
Birth to Age 5: Help Me Grow Home Visiting, Early Intervention & Early Childhood (3-5)
The time from birth to age 2 is key for children with developmental disabilities for their future development. It is important families have access to the services and resources they need to support their child and their future development. We also serve children ages 3-5 in our Early Childhood program.
Ages 6-23: School Age & Transition
This is a key time for children to learn skills like dependability, reliability and accountability and is an essential time for young adults. We will help you and your family navigate the complexities of today’s school and care systems and assist with the transition from school to adult life.
Ages 23 & Over
This is the time to continually ask, "What is important to me?” and "What does a happy life look life for me?" We will help you make plans to support those dreams, whatever they may be.
