How to make a Referral/Who is Eligible
Birth through age 2
To make a referral, please call (614) 656-3322. You can also make a secure online referral at http://bit.ly/ReferToHMG .You will need the child’s name, date of birth, address, parent’s name, and contact information.
Ages 3 through 5
If you would like to make a referral, please submit a secure online referral at the bottom of this page or contact Christy Payne at cpayne@knoxdd.com. Please also provide documentation of a qualifying diagnosis (intellectual or developmental disability) or evaluation showing developmental delays or established risk. If you do not have access to this documentation, complete the release section of the Referral Application and sign, this will allow the County Board to reach out to those listed for documentation. Please include in this list the name of your school district, physician, nurse practitioner, specialist, psychologist, etc.
Ages 6 and older
If you would like to make a referral, please submit a secure online referral at the bottom of this page or contact Christy Payne at cpayne@knoxdd.com. Please also provide documentation of a qualifying diagnosis (intellectual or developmental disability), verification of diagnosis can be provided using the Diagnosis Verification Form (below) or by providing signed documentation from a physician, psychologist, or certified nurse practitioner. Please include in this list the name of your school district, physician, nurse practitioner, specialist, psychologist, etc.
What are the qualifications?
Children birth through age 2
Children who have at least one developmental delay or an established risk for a developmental delay are eligible for our Early Childhood services.
Children age 3 through 5
Children age 3 through 5 who apply and provide verification of at least two developmental delays or an established risk are eligible to receive Early Childhood services.
Individuals age 6 and older
Individuals age 6 and older who apply and provide verification of a qualifying intellectual (diagnosed by a psychologist) or developmental disability (diagnosed by a physician or certified nurse practitioner) prior to age 22 may be eligible for County Board services. Knox County Board of DD utilizes the Children’s Ohio Eligibility Determination Instrument (COEDI) for determining eligibility for children ages 6 through 15 and the Ohio Eligibility Determination Instrument (OEDI) for those ages 16 and older. The COEDI or OEDI must determine limitations in at least three of the following areas of major life activities (economic self-sufficiency, self-care; receptive and expressive language; learning; mobility; self-direction; and capacity for independent living) to be found eligible.
