Provider Certification

In order to provide services to an individual on an IO, Level 1 and Self-Waiver or through local funding, providers must be certified through DODD (Department of Developmental Disabilities).

Below are step by step instructions on how to become certified, but the entire process must go through the dodd.ohio.gov website.  Please follow the instructions carefully. For questions about the certification process, please contact DODD directly at 1-800-617-6733 prompt #5.  They have an entire hotline set up specifically for certification questions.

This will be an overview of what needs to be done to get certified to provide services.



Initial Certification

This is an all-inclusive set of instructions that will guide you through the process from start to finish. Instructions on setting up the OH-ID, instructions on My Learning and step by step instructions for the PSM portal. Below are step by step instructions for completing the entire process.  Please read VERY carefully

Frequently added services for those providing HPC Services on the application are: Homemaker Personal Care, Homemaker Personal Care Transportation, Homemaker Personal Care-Participant Directed, Shared Living, Supported Living and Residential Respite.  This is a just the most frequent services chosen. Please review the service section carefully to see what best fits the services you are providing.

For specific question about the certification process please contact DODD directly at 1-800-617-6733 prompt #5.  DODD has a hotline set up to address any certification questions.

Below are instructions for beginning the initial application process.  Start with creating an OH-ID prior to “Application Instructions in PNM Portal” Please note there are instructions below to complete the NPI Number and Supplier ID which is needed for ALL applications.  Depending on the Service you are choosing to get certified for, you may also need the EVV Instructions as well.

All Providers are required to have an NPI number.
