Eligibility for Services

0 - 3 years old: Referrals for Early Intervention services are made through Help Me Grow program. Eligibility and plans for services are created in partnership with Knox County Board of Developmental Disabilities Early Intervention specialists.

To see if your child is eligible for services contact Help Me Grow Project Director, Theresa Vernon at 740-393-5720.

Age 3 and older: If you are interested in receiving supports through the Knox County Board DD you may:

  1. Come into the office and pick up the application information or call the Intake Department at 740-397-4607 and the information will be mailed to you. Review, print and complete forms that are located on this website and mail or bring the forms to the agency.

If you need help, the intake person will explain the steps to take to apply for enrollment and will assist you with completing the application and getting the documents you may need.

We will need:
• A completed application
• Documentation from a licensed professional (physician, psychiatrist, psychologist) stating your diagnosis & the date of onset. Documentation examples are a psychological evaluation, physical examination report, school IEP & MFE, or a letter from your physician.

Eligibility may be re-determined periodically to ensure that an individual is still eligible for services.

Family Support Groups

Many families have formed informal support groups around the specific diagnosis of their child. Some of these groups are online, communicating through forums such as Facebook, others meet in person. If you would like to start or join a family support group please contact your SSA for more details.
